Not sure if your plan covers our services? We can check your benefits before your first visit and explain your mental health coverage in clear, simple terms.

Please note: This is a courtesy verification and not a guarantee of payment. Your insurance plan makes the final determination when your claim is processed. You are responsible for any amounts not covered, including copayments, deductibles, coinsurance, non-covered services, and Hawaiʻi General Excise Tax (4.712%).

  • Copays, coinsurance, deductibles, and out-of-pocket maximums
  • Prior authorization or referral requirements

What We'll Need

To verify your coverage, please provide:

  • Full legal name and date of birth
  • Member ID, group number, and front/back of insurance card
  • Subscriber info (if you're not the primary member)
  • Consent for us to contact your insurance plan
  • Completed intake paperwork

Important to Know

Key things to keep in mind about your coverage and payments:

  • Late-cancel and no-show fees are not covered by insurance
  • You're responsible for any costs your insurance doesn't cover
  • We keep a credit card on file and may process estimated amounts 24–48 hours before or after claim processing
  • Good Faith Estimate available for uninsured or self-pay patients
